Zephyr Surgical Implants is proud to announce the first ZSI 475 FTM penile implant insertion in Germany. The procedure was performed by Acc. Prof. Dr. med. Gralf Popken, Head of Urology at the Ernst-von-Bergmann Clinic in Potsdam.
The patient initially had a three-piece inflatable penile implant for a cis man; however, it developed a leak and had to be replaced. Vascular vessels leading blood to the constructed penis were on the left hand side, so Dr. Popken performed a limited lateral incision at the base of the penis on the right. The previous implant was easily removed from this incision.
Penile dilation was performed using Brooks and Hegar dilators. Dilators of 20-25mm were not available so a clamp was used as a dilator to accommodate the new implant. The top of the penis was fibrotic. After this preparation the patient was ready to receive his ZSI 475 FTM penile implant.
Penile length was measured as 11cm + pubic bone space so a 12cm prosthesis was chosen.
First the reservoir was placed in the pelvis and filled with 50ml of saline, enough to fill a 12cm prosthesis and get a strong erection. The glans was initially too large to be inserted so the width was reduced by cutting the corona with scissors. After this it was able to reach the fibrotic top of the penis.
The proximal part was not sutured to the pubic bone as Dr. Popken considered that the fibrotic pouch made around the previous prosthesis was enough to stabilise the newly inserted ZSI 475 FTM, and that healing and post-operative fibrosis would further reinforce this.
Sufficient space was available in the scrotum to receive the ZSI 475 FTM pump. After connecting the pump with the reservoir an initial inflation test was performed to ensure function and quality of erection. The prosthesis found its place in the penis and a strong erection was achieved. When inflated the cylinders are 21-22mm in diameter. The thickness of the fat and skin must be added to this to determine the eventual diameter of the penis when erect.
The penis was returned to a flaccid state by pressing the deflation valve at the base of the pump and the surgery ended with suturing of the incision. The patient will now wait until healing is complete (6-8 weeks) before being able to use the new ZSI 475 FTM implant.
ZSI is the only European manufacturer of artificial urinary sphincters and of malleable and inflatable penile implants, as solutions for male urological problems (incontinence), sexual complications (Erectile Dysfunction), and gender identity procedures (prostheses for trans men and women).
